I Taxonomy and Characteristics

Chromobacterium violaceum belongs to the genus Chromobacterium within the family Neisseriaceae. It is the only species in this genus known to be pathogenic to humans. Morphologically, it is a facultative anaerobic Gram-negative rod with 1–4 lateral or subterminal flagella, in addition to a polar flagellum, and lacks a capsule.

Biochemically, the bacterium ferments glucose, maltose, and mannitol without gas production. A defining characteristic is the production of a distinctive violet pigment, violacein, although approximately 9–11% of clinical isolates may be non-pigmented variants. On nutrient agar, colonies appear smooth, slightly raised, and exhibit a metallic dark-purple sheen. The optimal growth temperature ranges from 30–35°C, reflecting its adaptation to warm environments.

Despite differences in pigmentation, both pigmented and non-pigmented strains exhibit comparable virulence, making detection and identification critical regardless of colony appearance.

II Ecology and Transmission

Chromobacterium violaceum is predominantly distributed in tropical and subtropical regions, first identified in the Amazon River basin. It is commonly found in soil and water environments, particularly in warm climates.

The bacterium infects individuals across all age groups, from infants to elderly patients. However, children, adolescents, and immunocompromised individuals are at higher risk due to reduced immune defenses.

Transmission primarily occurs through exposure to contaminated water or soil via open wounds or skin abrasions. Ingestion of contaminated water can also lead to gastrointestinal infection. Activities such as swimming, wading, or working in contaminated environments significantly increase exposure risk.

III Clinical Manifestations

Although infections are rare, Chromobacterium violaceum is associated with rapid disease progression and high mortality. Once infection occurs, the pathogen can disseminate through the bloodstream or local tissues, affecting multiple organ systems including skin, extremities, middle ear, urinary tract, bone marrow, and major organs such as the liver, spleen, kidneys, and lungs.

Patients typically present with fever, sepsis, skin lesions, abdominal pain, and localized abscess formation. A hallmark of infection is the development of multiple systemic abscesses. If not completely eradicated, recurrence is common.

Severe cases may progress to septicemia within 48 hours after bloodstream invasion, with reported mortality rates reaching up to 56%. This highlights the importance of early detection and prompt intervention.

IV Laboratory Diagnosis and Molecular Detection

Laboratory identification of Chromobacterium violaceum traditionally relies on culture characteristics and biochemical testing. However, due to the presence of non-pigmented variants and rapid disease progression, molecular diagnostic methods are increasingly important.

PCR and real-time qPCR assays provide highly sensitive and specific detection of Chromobacterium violaceum, enabling early diagnosis even in cases with low bacterial load or atypical presentation. These methods are particularly valuable for clinical microbiology laboratories and environmental monitoring applications.

V Treatment and Management

Effective antimicrobial therapy is essential for managing Chromobacterium violaceum infections. The bacterium is generally susceptible to imipenem, fluoroquinolones, gentamicin, tetracyclines, and trimethoprim-sulfamethoxazole, which are considered first-line treatment options.

Resistance has been observed against penicillins and narrow-spectrum cephalosporins, and these should be avoided in treatment. The efficacy of aminoglycosides and third-generation cephalosporins may vary and should be guided by antimicrobial susceptibility testing.

Due to the tendency for systemic abscess formation and persistent infection, treatment typically involves a prolonged, staged approach. Initial intensive therapy with intravenous antibiotics for 2–3 weeks is followed by an extended oral maintenance phase lasting 2–3 months to prevent recurrence.
