Background: Hantavirus and Emerging Threats

What is Hantavirus?
Hantaviruses are negative-sense, single-stranded RNA viruses in the family Bunyaviridae, genus Hantavirus. They are zoonotic pathogens primarily transmitted to humans through contact with infected rodent excreta (urine, feces, saliva). Hantavirus infection can cause two severe clinical syndromes:
1. Hemorrhagic Fever with Renal Syndrome (HFRS)
Primarily associated with Old World Hantavirus species (Hantaan, Seoul, Dobrava). Geographic distribution: Europe, Asia, parts of Russia. Case fatality rate: 1–15% depending on species and access to supportive care.
2. Hantavirus Pulmonary Syndrome (HPS)
Caused by New World species, most notably Andes virus (ANDV). Geographic distribution: South America (primarily Argentina, Chile). Case fatality rate: 35–50%, with rapid progression to respiratory failure.
Andes Virus (ANDV): The Outbreak Strain
ANDV is notable for human-to-human transmission capability — the only known Hantavirus species with documented person-to-person spread. This characteristic makes ANDV a particular concern for outbreak escalation in healthcare and enclosed settings (cruise ships, dormitories, hospitals).
A cluster of ANDV cases aboard a transatlantic cruise ship (2023–2024) generated international public health attention, demonstrating the potential for rapid transmission in high-density environments. Mortality rate in the cruise ship cluster exceeded 50%, underscoring the clinical severity.
